FULL-TIME INSTRUCTIONAL PERSONNEL
REQUEST FOR TUITION WAIVER
The Tuition Waiver program is available to all full-time instructional personnel that have completed 90 days of continuous employment. This benefit is available to the employee and their eligible dependents. The maximum number of credits that will be approved for the employee is six (6) credits per MDC semester (Fall, Spring, Summer A and B combined), as per Faculty Contract. This benefit program does not include the cost of any application fees, lab fees, special fees or book(s).
I certify that I have completed 90 days of full-time continuous employment at Miami Dade College and understand that as an employee I am eligible to register for a maximum of six (6) credits as stated above.

After Obtaining supervisory approval, this form must be delivered to the Financial Aid Office to complete the Tuition Waiver Request process.
NOTE: Employees must have PRIOR supervisory approval to attend class(es) during work hours. Employee must complete and provide the Authorization for Employee to Register for Courses During Normal Workday form to the supervisor for approval.
