REQUEST FOR TUITION WAIVER


The Tuition Waiver program is available to all full-time, PEC, PENC and SNE employees that have completed 6 months of continuous, full-time employment.  This benefit is available to the employee and their eligible dependents. The maximum number of credits that will be approved for the employee is eight (8) credits per MDC semester (Fall, Spring, Summer A and B combined). This benefit program does not include the cost of any application fees, lab fees, special fees or books.


I certify that I have completed six months of  full-time continuous employment at Miami Dade College and understand that as an employee I am eligible to register for a maximum of eight (8) credits as stated above.

After Obtaining supervisory approval, this form must be delivered to the Financial Aid Office to complete the Tuition Waiver Request process.


NOTE: Employees must have PRIOR supervisory approval to attend class(es) during work hours. Employee must complete and provide the Authorization for Employee to Register for Courses During Normal Workday form to the supervisor for approval. This form can be requested by contacting the Human Resources Request Help Line at x7-0616 or can be printed from the Human Resources web site at http://www.mdc.edu/hr
