Supervisor Checklist for Separating Employees
Supervisors must complete the "Supervisor Checklist for Separating Employees" form for all employees (full-time and part-time) who are separating from the College (resigned, retired, on long-term leave or terminated). The form may also be used for employees transferring from one department to another. The form must be completed prior to the separation date in order to terminate security access and collect College property as warranted.
Please open the form (see link below) and search for the employee by entering the first few letters of the employee's first and last name. The following fields will automatically be populated: Employee Name, Department, Telephone Number and Network ID. Next you will select the appropriate campus location and president/provost from the drop down boxes.
Then you must review each item on the list and select the appropriate response. Print a hard copy of the completed form, have it signed by the Bursar, and attach it to the Exit Request for Personnel Action and send via administrative channels to the Division of Human Resources. Lastly, press "submit" and the form will be forwarded to CCIR, Human Resources and the respective Provost/Campus President.
