 PLANT MAINTENANCE WORK REQUEST |
|
The Plant Maintenance Work Request is a three page carbonless form that must be neatly printed and completed. These forms may be obtained through the Campus Services Department at extension 7-6260. Plant Maintenance Work Requests are to be used solely for the following: |
- Key Requests (must include the Social Security and MDID number of the person which the key will pertain to)
- Hanging of pictures and bulletin boards
- Any type of service or repair that requires the use of tools

*A plant maintenance work order must be signed by the requestor and the head of the department, and then submitted to the Director of Administrative Support Services after which it will be sent to the Campus Services Director.
*Issues involving the picking up of boxes after there preparation should be e-mailed to mhalloran@mdc.edu , with a carbon copy sent to rpaz@mdu.edu; after which they will be submitted to Campus Services and then faxed to Pedro Gomez at extension 7-1279. Boxes will be kept in temporary storage until pick up on Tuesday or Thursday.

The Plant Maintenance Trouble Report is an electronic form which is sent directly to the main Maintenance Office in the North Campus. The form located on the web at: http://www.mdc.edu/
mdc/treport.htm. It is suggested that for your records, you print out a copy of the confirmation shown on the screen once the trouble report has been submitted. Aside from using the online form, you may also call 7-1710. The Plant Maintenance Trouble Report is to be used solely for the following: |
- Air Conditioning (too cold or too hot in a room or office)
- Lights out
- Squeaky Doors
- Electricity out

The Telecommunications Work Request is an electronic form which is sent directly to the Telecommunications Offices which are located at the Wolfson Campus. The form is located on the web at http://www3.mdc.edu/telecom/workreq.asp. It is suggested that for your record, you print out a copy of the confirmation shown on the screen once the request has been submitted. Aside from using the online form, you may also call 7-3300. The Telecommunications Work Request is to be used solely for the following: |
- Telephone not working properly
- Broken telephone
- Requesting additional telephone lines
- Requesting the move of telephone lines
- Any other service needed regarding the telephones
