Employee Guidelines for Long Distance Call Detail Reports

Each employee with long distance
telephone service should review his/her Call Detail Report (CDR) to
verify that calls and faxes on the report are calls related to College
business. All long distance calls and faxes must be logged on the Departmental
Long Distance Call and Fax Log. The report should then be signed
and returned to his/her supervisor or designee within thirty (30) days
of receipt.

Long
distance calls and faxes of a personal nature can only be made from
the College telephone system in the case of an emergency. The
College must be reimbursed for each long distance personal emergency
call and fax. Employees who make personal long distance calls and
faxes using College long distance service must submit a Personal
Long Distance Call and Fax Remittance Form with a check or
money order, payable to Miami Dade College, along with the
relevant page(s) of the CDR. Each personal call should be marked with
a "P" in the left margin of the CDR page adjacent to the
call information. The Form and CDR page(s) should be sent to: Network
& Telecommunications Services, Kendall Campus, Room 9158.

All long distance calls and faxes that
are not authorized by you must be reported to Network &
Telecommunications Services using the Unauthorized
Call and Fax Report Form. A copy of the relevant CDR page(s)
marked with a "U" in the left margin of the CDR page
adjacent to the call information should accompany this Form. This form
requires the signature of the supervisor and should be sent to:
Network & Telecommunications Services, Kendall Campus, Room 9158.

The MDC
Departmental Long Distance Call and Fax Log is available to
facilitate record keeping.

Departments are responsible for storing
signed CDR reports and logs for their area and furnishing them upon
request for State and local auditors. These reports and logs are
required to be stored for a period of three (3) years.

Use of Long Distance Telephone Services
Summary
- Only authorized
long-distance calls (i.e., those related to College business) allowed.
- In the case of
a personal emergency, the caller must reimburse the College.
- Long
distance calls and faxes that you have not authorized must be reported
to Network and Telecommunications Services.
- Third party calls
may not be placed from or billed to a College telephone.
- Collect calls
may not be accepted
