Supervisor Guidelines for Long Distance Call Detail Reports


  1. Call Detail Reports for each Campus and District areas will be sent to the Campus Deans for Administration, the Foundation and Vice Provosts or their designees for distribution to departments once a month.

  2. Each employee with long distance telephone service should review his/her Call Detail Report (CDR) to verify that calls and faxes on the report are related to College business. All long distance calls must be logged on the Departmental Long Distance Call and Fax Log. The report should then be signed by the employee and returned to his/her supervisor or designee within thirty (30) days of receipt.

  3. Long distance calls and faxes of a personal nature can only be made from the College telephone system in the case of an emergency. The College must be reimbursed for each long distance personal emergency call or fax. Employees who make personal long distance calls or faxes using College long distance service must submit a Personal Long Distance Call and Fax Remittance Form with a check or money order, payable to Miami Dade College, along with the relevant page(s) of the CDR. Each personal call should be marked "P" in the left margin of the CDR page adjacent to the call information. The Form and CDR page(s) should be sent to: Network & Telecommunications Services, Kendall Campus, Room 9158.

  4. All long distance calls and faxes not authorized by you must be reported to Network &Telecommunications Services using the Unauthorized Call and Fax Report Form. A copy of the relevant CDR page(s) marked with a "U" in the left margin of the CDR page adjacent to the call information should accompany this Form. This form requires the signature of the supervisor and should be sent to: Network & Telecommunications Services, Kendall Campus, Room 9158.

  5. The MDC Departmental Long Distance Call and Fax Log is available to facilitate record keeping.

  6. Departments are responsible for storing signed CDR reports and long distance logs for their area and furnishing them upon request for State and local auditors. These reports and logs are required to be stored for a period of three (3) years.
