Room Request Form
This form is for Medical Campus faculty and staff to submit a request to reserve one or both of the testing department rooms for computerized testing.

Test Banking Forms

Scantron Request Form 
This form is for Medical Campus faculty to request a supply of various types of Scantrons.

Revision Processing Form 
This form is for Medical Campus faculty to submit a request to make changes to an existing test bank. Please allow 3 business days to process this request.

Score Request Form 
This form is for Medical Campus faculty to submit a request to set up a test bank for a test or quiz. Faculty must also provide a Scantron answer key. The key can be scanned and attached to the online form, dropped off in person to Gabby in the Testing Center, or dropped off with a copy of the online form confirmation email in the “Test Banking” dropbox outside of the HPS lab. Please allow 3 business days to process this request.

Electronic Grade Book Request Form 
This form is for Medical Campus faculty to request the creation of a class grade book. Please note that each semester a new grade book must be created; therefore, a new request must be submitted each semester. 
*Please note that you will need to complete a form for each course reference number.
