In order to apply by mail students must include their date of birth, street address, phone number, email address and the name of the person who referred them—if any—on an index card and mail it to GotChosen at 12472 Lake Underhill Rd. #397, Orlando, Fla. 32828.
The scholarship advertises that it requires “No GPA—No essay—No long form to fill” to apply.
Scholarship deadline is Sept. 30.
