School of Health Sciences Discipline Selection

Congratulations on your provisional approval to progress to discipline courses with the School of Health Sciences. Please follow the steps below in order to complete the progression process. Program Orientation will be held on Tuesday, July 9th from 9:30 am until 1:00 pm in Room 3101 (Medical Campus). We will see you there! Failure to attend Orientation may cause you to forfeit your seat. 

Complete All Steps Below:

  1. Response
    • To submit your Response please visit the link in Progression email you received.
  2. Level Two Background Screening
    • Complete and clear the Interstate Background Research (IBR) Level Two criminal background screening. The cost for the background screening is approximately $63.00. You will need to schedule an appointment for the background screening.
  3. Student Health Record
    • The **Student Health Record must be taken to a health care provider (Physician, Nurse Practitioner, or Physician Assistant) to be completed.
      **All necessary guidelines regarding the completion of the student health record can be found on the front page of the student health record.
  4. Titers/Tests/Vaccines - Please follow the instructions in the Student Health Record.Your Program Director will further discuss these requirements with you during your orientation approximately 8 weeks before the start of the semester.
    • Provide supporting documentation for the following:
      1. Proof of immunity(blood titer lab tests), from Varicella (Chicken Pox), Mumps, Rubeola (Measles), and Rubella (German Measels) within the last 5 months from the start of the program/semester.
      2. Two current (within 2 months from the start of the program/semester) consecutive TB skin tests (at least 7 days apart) or the QuantiFERON test
      3. A minimum of a 10-panel drug screening results
      4. Proof of immunity (blood titer lab tests) againstHepatitis B or the vaccine series within the last 5 monthsfrom the start of the program/semester.
      5. Tdap vaccination within ten (10) years
  5. American Data Bank (Complio)
    • You must create an account IMMEDIATELY with American Data Bank. Please choose "School of Health Sciences" - and select your program.
    • You must submit the requested documents mentioned in Steps 2, 3, 4, and 5, to American Data Bank using the Complio system
    • It is recommended you order a 10-panel drug screening from American Data Bank in the amount of $35.
    • You are also required to fulfill the additional requirements contained within the Complio system.
      • American Data Bank's "Complio" is an online system used by MDC's Medical Campus for tracking immunizations and other requirements. The cost is $24 for one year or $45 for two years. We suggest you only purchase the subscription for one year for now, further instructions will be provided by the program.
  6. Must attend the following orientation. Carefully plan your calendar accordingly.
    • Program Orientation will be held on Tuesday, July 9th from 9:30 am until 1:00 pm in Room 3101 (Medical Campus).
  7. Vocational Application
    • Please also note that in addition to the regular college admissions application you have already completed, you must also complete the vocational application. Please complete this vocational application before attending the orientation session. This application must be completed online (select credit application, then, new application, continue, enter your email address; answer the questions; enter the term; then click on "degree student to complete a program/degree"; click on Career Technical Certificate; select your program from the list; complete the remaining steps.) Failure to complete the vocational application will cause problems with registration.
