School of Health Sciences Discipline Selection

Congratulations on your provisional approval to progress to discipline courses with the School of Health Sciences. Please follow the steps below in order to complete the progression process. You must complete all of the steps at least 2 days prior to your scheduled Orientation.

Complete All Steps Below:

  1. Response and Pay
    • To submit your Response please visit the link in Progression email you received.
    • $800 Seat Deposit must be received by Bursars office within 5 business days of the date of your provisional acceptance letter. 
    • You must complete the Program Acceptance & Deposit Agreement Form and send all requested information to jhernan7@mdc.edu
  2. Level Two Background Screening
    • Complete and clear the Interstate Background Research (IBR) Level Two criminal background screening. The cost for the background screening is $62.25. You will need to schedule an appointment for the background screening.
  3. Student Health Record (Due Monday, April 1st 2024)
    • The **Student Health Record must be taken to a health care provider (Physician, Nurse Practitioner, or Physician Assistant) to be completed.
      **All necessary guidelines regarding the completion of the student health record can be found on the front page of the student health record.
    • If you have received the COVID-19 Vaccine please submit copy of your vaccine card with the student Health Record form.
  4. Titers/Tests/Vaccines (Due Monday, April 1st, 2024)
    • Provide supporting documentation for the following:
      1. Varicella (Chicken Pox), Mumps, Rubeola (Measles), and Rubella (German Measles) blood titer lab results
      2. Two current (within the past 3 months) consecutive TB skin tests (at least 7 days apart) or the QuantiFERON test
      3. A minimum of a 10-panel drug screening results
      4. Hepatitis B vaccine series
      5. Tdap vaccination within ten (10) years
  5. American Data Bank (Complio)
    • You must create an account IMMEDIATELY with American Data Bank.
    • You must submit the requested documents mentioned in Steps 2, 3, 4, and 5, to American Data Bank using the Complio system. Step 2 must be completed at least 2 days prior to orientation and Steps 3, 4, and 5, must be completed by Monday April 1st, 2024. You are also required to fulfill the additional requirements contained within the Complio system. This includes ordering a 10-panel drug screening from American Data Bank in the amount of $35.
    • The MDC PA program will only accept a 10 -panel drug screening from American Data Bank.
      • American Data Bank's "Complio" is an online system used by MDC's Medical Campus for tracking immunizations and other requirements. The cost is $24 for one year or $45 for two years. We suggest you only purchase the subscription for one year for now, further instructions will be provided by the program.
    • You will receive your first semester class schedule at your Orientation following determination that you are "fully compliant" within Complio.
  6. Must attend the following mandatory orientation. Carefully plan your calendar accordingly.
    • Information regarding orientation will be emailed to you by the program. Please be prepared to attend two orientations (One for MDC PA program & the other for NSU Master's program).

    • Welcome to Nova Southeastern University and the Master of Health Science program! As you may know, NSU is the 7th largest private University in the United States and is known for quality on-campus and online Bachelors, Masters, and Doctoral programs. As the demand for master level health care providers, educators, and administrators continues to grow, the M.H.Sc. curriculum will provide you with a learner-centered educational experience that emphasizes critical-thinking, decision making, leadership, and the concept of the scholar-practitioner and lifelong learning. 

      Admission Requirements:

        All students applying to the NSU M.H.Sc. Program must do all the following by March 1st, 2024

      • Complete an NSU M.H.Sc. Program application form with prevalent application fee: NSU M.H.Sc. Program Application Form 
      • Provide evidence of acceptance into the MDC PA Program
      • Provide evidence of a minimum cumulative grade point average (GPA) of 3.0 or higher on a 4.0 scale
      • Provide official college, certificate and/or diploma-based transcripts from all undergraduate and graduate institutions attended, sent directly from the institutions to NSU
      • Send transcripts and all required documents to:

      • Applicant is responsible for contacting one of the evaluation services listed here. The evaluation must be course by course including a cumulative GPA The official evaluation must be sent directly from the evaluation service to the NSU EPS address shown above for all documents:

  9. Helpful Information
    • Students are encouraged to apply for financial aid and scholarships as early as possible. We recommend applying for financial aid through NSU as this may provide higher financial assistance.

Note: Failure to complete the entire above requirements by the stated deadline forfeits your seat in this program.
