The following is an assessment protocol that can be used to collect data that can guide a disability service provider or a researcher in better understanding students as complex individuals first known by their strengths as well as the effects of disability. The Protocol may be copied and printed for research by other institutions.
Assessment Interview Protocol

The following questions may be revised depending upon the comfort level and the communication skills of the student being interviewed. Many additional questions can be pursued depending upon the time and interest of the interviewer and the interviewee. Responses should be recorded and attached on separate sheets for review. Immediately or shortly following the interview, the interviewer should begin to list key points that highlight student strengths that can be used to build program support. The report should be extended over each semester as new information is gained, and adjusted for changes that come about in the student herself or himself.

4. What do you think have been the major life influences that brought you to this point?
5. What was high school like? Did you graduate?
6. What do you think are your achievements and successes in life so far?
