School of Health Sciences Discipline Selection

Congratulations on your provisional approval to progress to discipline courses with the School of Health Sciences. Please follow the steps below in order to complete the progression process. You must complete steps 1 and 2 at least 5 days prior to your scheduled Orientation.

The Histologic Technology Associate in Science Program Orientation will be held Monday, July 10, 2023, at 8:30 am at the Medical Campus Room 3102. You must be in attendance for the entire duration of the orientation.

Complete All Steps Below:

    • To submit your Response please visit the link in Progression email you received.
  2. State of Florida Board of Clinical Laboratory Trainee License application
  3. American Data Bank (Complio)
    • American Data Bank's "Complio" is an online system used by MDC's Medical Campus for tracking immunizations and other requirements. The cost is $24 for one year or $45 for two years. We suggest you only purchase the subscription for one year for now, further instructions will be provided by the program.
    • You will receive your first semester class schedule at your Orientation following determination that you are "fully compliant" within Complio.
    • You must create an account immediately with American Data Bank.
      • Once your account is created, register for School of Health Sciences Histologic Technology
      • 3a. You need to upload the 1st year paperwork listed below, before March 30, 2023
        • State of Florida Board of Clinical Laboratory Personnel Trainee License
        • Emergency Contact Form
        • Signed Histologic Technology program laboratory safety memo
        • Signed Univrom affidavit, affective and psychomotor domain explanations
        • Signed Histologic Technology Program Practicum Agreement
        • Signed Confidentiality statement
      • 3b. 2nd year paperwork will not be populated until the beginning of your second year. The 2nd year paperwork that must be uploaded during the spring of your 2nd year includes:
        • The **Student Health Record must be taken to a health care provider (Physician, Nurse Practitioner, or Physician Assistant) to be completed.
        • Background check
          • Level Two Background Screening-(During spring of 2nd year in program)
            • Complete and clear the Interstate Background Research (IBR) Level Two criminal background screening. The cost for the background screening is $57. You will need to schedule an appointment for the background screening.
        • Drug screening (10-panel drug screening from American Data Bank in the amount of $35.)
        • Titers/Tests/Vaccines (During spring of 2nd year in program)
          • Provide supporting documentation for the following:
            • Varicella (Chicken Pox), Mumps, Rubeola (Measles), and Rubella (German Measles) blood titer lab results
            • Two current (within the past 3 months) consecutive TB skin tests (at least 7 days apart) or the QuantiFERON test
            • A minimum of a 10-panel drug screening results
            • Hepatitis B vaccine series or signed declination form
            • Tdap vaccination within ten (10) years
  4. Must attend the following mandatory orientation. Carefully plan your calendar accordingly.
    • Mandatory orientation is on Monday, July 10th from 8:30 AM to 2:30 PM in person at the Miami Dade College Medical Campus Room 3102.
    • You must be in attendance for the entire duration of the orientation!
  5. Helpful Information
    • Students are encouraged to apply as early as possible for Financial Aid and to apply for Scholarships. Please visit the financial Aid and scholarships websites for additional information.

Note: Failure to complete the entire above requirements by the stated deadline forfeits your seat in this program.
