Physical Therapist Assistant

Congratulations on your provisional approval to progress to discipline courses with the School of Health Sciences. Please follow the steps below in order to complete the progression process.

You must complete step 1 by October 20th, and steps 2, and 3 by October 30th, 2023.

Please note that steps 5, 6, and 7 does not need to be completed until Wednesday, January 31, 2024.

Complete All Steps by the Date Indicated:

  1. Response
    • Submit our Response Form by October 20th, 2023. Please visit the link provided in the Progression email you received.
  2. Level Two Background Screening
    • Complete and clear the Interstate Background Research (IBR) Level Two criminal background screening. The cost for the background screening is $57. You will need to schedule an appointment for the background screening ASAP.
  3. American Data Bank (Complio)
    • You must create an account IMMEDIATELY with American Data Bank.
    • Submit the requested documents to American Data Bank using the Complio system.  
    • You must also fulfill the additional requirements contained within the Complio system including read the PTA Policy and Procedures Handbook and Lab Safety Manual and sign the required pages before orientation on Wednesday, November 1st, 2023.
    • Print the signature pages only and bring to orientation on Wednesday, November 1st, 2023.
      • American Data Bank's "Complio" is an online system used by MDC’s Medical Campus for tracking immunizations and other requirements. The cost is $24 for one year or $45 for two years. We suggest you purchase the two-year subscription as you will utilize this for your entire time in the program.
        • It is strongly recommended you review the videos in the MDC Complio Instruction Sheet. If additional information is needed, please review the Complio Help Guide.
        • We will register you for your first semester classes and you will receive your schedule at orientation following determination that you are “fully compliant” within Complio.
  4. Must attend the following mandatory orientation. Carefully plan your calendar accordingly.
    • Check-in starts promptly at 8:00 am. Continental breakfast will be provided. Lunch will be on your own.
    • Failure to attend orientation will result in relinquishment of your seat to another applicant.
  5. Student Health Record (Note: This is due on Wednesday, January 31, 2024)
    • The **Student Health Record must be taken to a health care provider (Physician, Nurse Practitioner, or Physician Assistant) to be completed by Wednesday, January 31, 2024.
      **All necessary guidelines regarding the completion of the student health record can be found on the front page of the student health record.
  6. Titers/Tests/Vaccines (Note: This is not due on Wednesday, January 31, 2024)
    • Provide supporting documentation for the following:
      1. Varicella (Chicken Pox), Mumps, Rubeola (Measles), and Rubella (German Measles) blood titer lab results
      2. Two current (within the past 3 months) consecutive TB skin tests (at least 7 days apart) or the QuantiFERON test
      3. A minimum of a 10-panel drug screening results
      4. Hepatitis B vaccine series
      5. Tdap vaccination within ten (10) years
      6. You must also order a 10-panel drug screening from American Data Bank in the amount of $35.
  7. CPR/BLS Card (Note: This is not due on Wednesday, January 31, 2024)
    • Provide and maintain a current copy of a CPR/BLS card issued only by the American Heart Association that is valid throughout the program. If you need to complete this 6 hour CPR/BLS course you can call (305) 237-4177 or check online for the schedule of classes offered at Miami Dade College.
  8. Prior to the first day of classes (Wednesday, January 3, 2024) for the PTA Program:
    • Pay all outstanding tuition.
    • Purchase required books and uniform (highly recommend doing this on orientation day).
    • Have a laptop (This is MANDATORY) View the laptop minimum requirements.
  9. Helpful Information
    • Students are encouraged to apply as early as possible for Financial Aid and to apply for Scholarships. Please visit the financial aid and scholarships websites for additional information.

Note: Failure to complete all the above requirements by the stated deadline and attend orientation forfeits your seat in this program.
