IE Service Request

The Office of Institutional Effectiveness (IE) has provided this form to handle and track service requests for internal and external purposes. We appreciate your use of this form for all IE service requests.

Our office strives to provide you with timely, accurate, and clearly presented information for a variety of uses in a timely manner. For this reason, it is very important that requests are made as far in advance as possible, preferably at least three to four weeks in advance of the date you require the information. Occasionally undue delays may occur based on available resources and workload. You will be notified if your request cannot be accommodated within the requested time-frame. Please be assured that we will work with you when emergency requests arise; however, so that all customers enjoy the same level of service and projects/requests are prioritized appropriately.
