To be eligible for admission to the Phlebotomy Program at the Medical Center Campus, you must meet these criteria.
- Test of Adult Basic Education must be completed and passed before program completion. The exit levels required are:
- Reading : 10.0 Math: 9.0 Lang: 10.0
- A physical examination is required of all students.
- Proof of immunization must be shown, including Hepatitis B. In addition, students must present a current CPR-BLS card and HIV/AIDS certificate before they can enter clinical sites.
- Students must apply for graduation prior to program completion. Apply at the Medical Center Campus Room 1354.
- Students should check their individualized degree audit report to determine the specific graduation policies in effect for their program of study for the year and term they enter Miami Dade. This outline includes current graduation requirements.
- Complete a Miami Dade College MDC) Application for Admission or Readmission to Vocational Credit Courses and Programs and submit to the Admissions Office at the Medical Center Campus (Room 1203) or at any Web at www.mdc.edu.

- Request former school and/or non-MDC Colleges to forward transcripts (including high-school and degree) to the Admissions Office, before the admission review deadline.
- Complete the Test of Adult Basic Education (TABE) in the Testing Department (Room 1303) or any Testing Department at MDC campuses. Call 305-237-4275 for a testing schedule. Students who have earned an Associate degree or higher, or who have passed the C.P.T. on their first attempt, are exempted from the T.A.B.E. Test.
- Visit the Vocational Certificate Student Resource Center (VCSRC) to complete the following steps:
- Have your TABE score report or a copy of a non-MDC score report or a copy of a non-MDC score report evaluated for meeting program admission requirements.
- Receive advisement regarding T.A.B.E. exemption , course requirements, career choices, and course selections.
- l Submit evidence of previously earned academic/technical healthcare certifications and experience to receive credits for HSC 0001.
- Complete an Application for Program Selection.
- Or, call 305-237-4374 for more information and program/admission advisement from an advisor in the (VCSRC) in Room 2104.
Completion of this checklist alone does not guarantee acceptance into the program. Students are selected on a competitive basis
