Congratulations on your provisional admission to the School of Nursing. Please follow the steps below in order to complete the admission process. The response form must be returned no later than the date indicated in your email. Please remember that you must complete all of the steps no later than indicated. Do not wait until the last minute.
Follow Steps Below:
1. Bursar's Office
- Pay the $100.00 non-refundable acceptance deposit at the Bursar's Office at the Medical Campus. Attach the receipt to the PROGRAM ACCEPTANCE DEPOSIT TRANSACTION RECORD (PDF) form, which must be submitted to the New Student Center (Room 1113 at Medical Campus) no later than the date indicated in your email. This fee will be credited towards your tuition after the 100% refund date for your first term in program courses.
2. Response Form
- Complete the Response Form no later than the date indicated in your email.
- Once you accept a seat in one option, you must remain in that option through program completion; you will not be allowed to switch between options.
3. Criminal Background Check
- Successfully complete and clear the MDC vendor level two criminal background check and the Agency for Health Care Administration (AHCA) screening prior to the start time of your orientation. You will need to schedule an appointment for the background check and the AHCA screening.
- The MDC vendor level two criminal background check must be completed and submitted to *American Data Bank (Complio) before the start time of your orientation. The renewal period is annually or as requested.
- The results of the AHCA screening will be released directly to MDC clinical partners when you are assigned to a facility for clinical rotations. The renewal period is every 5 years.
4. Student Health Record
- The **Student Health Record must be taken to a health care provider (Physician, Nurse Practitioner, or Physician Assistant) to be completed. It must be completed and submitted to *American Data Bank (Complio) before the start time of your orientation.
- Student health screening services are available at these providers. You can also visit your personal physician for these services.
**ALL NECESSARY GUIDELINES REGARDING THE COMPLETION OF THE STUDENT HEALTH RECORD CAN BE FOUND ON THE FRONT PAGE OF THE STUDENT HEALTH RECORD.
5. Titers/Tests/Vaccines
- Provide supporting documentation for the following:
- Varicella (Chicken Pox), Mumps, Rubeola (Measles), and Rubella (German Measles) blood titer lab results
- Two current (within the past 3 months) consecutive TB skin tests (at least 7 days apart) or the QuantiFERON test
- A minimum of a 10-panel drug screening results
- Hepatitis B vaccine series
- Tdap vaccination within ten (10) years
- These items must be completed and submitted to *American Data Bank (Complio) before the start time of your orientation.
6. CPR/BLS Card

- The day and time of the mandatory orientation will be provided soon.
8. *American Data Bank (Complio)
- American Data Bank's "Complio" is an online system used by Medical Campus for tracking immunizations and other requirements. The cost is $24 for one year or $45 for two years. As you have been requested in the steps above, you must submit the requested documentation to American Data Bank using the Complio system at http://www.mdccompliance.com/index.html before the start time of your orientation. It is strongly recommended you review the videos in the MDC Complio Instruction Sheet. If additional information is needed, please review the Complio Help Guide.
- Registration for classes will be permitted only after you are “fully compliant” within Complio and have completed orientation.
9. Helpful Information
Note: Failure to complete the entire above requirements by the stated deadline forfeits your seat in this program.
